Referrals
For Physicians and Urologists
We welcome referrals from physicians and urologists for patients seeking diagnostic services:
- Prostate biopsies: TRUS, MRI Fusion, and Transperineal
Or minimally invasive treatments for:
- Prostate cancer: HIFU, NanoKnife (IRE)
- BPH: Rezum, iTind, Optilume
- Men’s health procedures: vasectomy, cosmetic or adult ritual circumcision
How to Submit a Referral
- Send the completed form via fax: +1 (416) 979-9002
- Our team will review the referral and schedule a consultation as appropriate
For Self-Referred Patients
At UIC, we only see new patients considering prostate biopsy, or minimally invasive, outpatient treatments for prostate cancer, BPH, erectile dysfunction, vasectomy, and cosmetic or adult ritual circumcision. These treatments are not covered by OHIP or other provincial health plans.
(Exceptions: Vasectomy and prostate biopsies – these procedures are covered by the provincial health plan, but a facility fee applies.)
Eligibility Criteria
- If you are currently under the care of a hospital-based urologist for an OHIP-covered treatment, you must continue your care through that system. However, if you are exploring minimally invasive alternatives, such as focal therapy for prostate cancer or non-surgical BPH treatments, we welcome you to contact us to discuss your options.
- We treat patients who are seeking prostate biopsy, focal therapy for prostate cancer, non-invasive BPH procedures, ICI therapy, vasectomy, or adult circumcision.
Consultation Format
Prostate Cancer (IRE/HIFU):
Virtual consultation (video call) OR in-person – no referral needed.
BPH Treatments (Rezum, iTind, Optilume):
Virtual consultation (video call) OR in-person – referrals strongly preferred.
ICI Therapy, Vasectomy, and Cosmetic or Adult Ritual Circumcision:
Virtual consultation (video call) OR in-person – referrals preferred but not required.
Due to the volume of inquiries, we can only respond to patients who meet medical and administrative criteria for a procedure at our clinic. We will respond within 5 business days to discuss whether you are eligible for one of our treatments, to arrange a consultation, or provide further guidance.
