The Process
1. Consultation & Assessment
We begin with a review of your medical history, PSA levels, prior imaging, and any previous biopsy results to determine whether an MRI-guided biopsy is appropriate. We may prescribe an antibiotic course that should be started on the night before the biopsy.
2. Pre-Biopsy MRI
A multiparametric MRI (mpMRI) is performed to locate and evaluate any suspicious areas within the prostate. These MRI images are used to guide the biopsy session with precision.
3. Biopsy Procedure
During the biopsy, real-time ultrasound is fused with the pre-acquired MRI images to guide the needle precisely to the targeted areas. The biopsy is performed transperineally – through the skin between the rectum and scrotum – allowing for improved accuracy and lower infection risk.
4. Post-Biopsy Care
You’ll receive instructions for recovery and scheduling follow-up appointments to discuss results and potential treatment options.

Specific instructions will be provided during your consultation, which may include guidelines on medications, diet, and activity restrictions before the procedure.
Most patients experience minimal discomfort and can resume normal activities shortly after the procedure. Detailed post-care instructions will be provided to ensure a smooth recovery. It’s important to complete the prescribed antibiotic course to reduce the risk of infection.
For best recovery:
- Avoid strenuous activity and bike riding for 24-48 hours
- Refrain from sexual activity for one week
Mild bleeding is common after a biopsy:
- Light bleeding from the front or back may last for 4-5 days
- Slight blood in semen may persist for up to 8 weeks
Discomfort is typically mild and can be managed with over-the-counter pain relief (e.g., Extra Strength Advil or Tylenol).
There is a very small risk of developing a urinary tract infection (UTI) or, rarely, sepsis. If you experience fever, chills, shaking, or rigors, go to the nearest Emergency department immediately and inform them that you’ve recently had a prostate biopsy.
Your results will be ready in approximately 3 weeks. You must arrange a follow-up appointment with your treating urologist to receive the results.
